During these unprecedented times, we are asking for the cooperation of all our patients. Please review and abide by the following guidelines and protocols.

  1. Arrive for appointments 10-15 minutes early and call when you arrive. If you do not have a cell phone, please call when you leave home.

  2. Please always wear a mask while inside the office and not receiving treatment.

  3. Payments are being received by Visa, Debit, and Mastercard only at this time. No cash or cheques please.

  4. Please use hand sanitizers upon entry and exit of the office.

Please review these COVID-19 screening questions before EACH visit, and call the office if you answer "yes" to any.

  1. Have you travelled outside of Canada in the last 14 days?

  2. Have you tested positive for COVID-19, or had close contact with a confirmed case of COVID-19 (without wearing appropriate PPE)?

  3. Do you have any of these symptoms?

    • Fever

    • New onset of cough

    • Worsening chronic cough

    • Shortness of breath

    • Difficulty breathing

    • Sore throat

    • Difficulty swallowing

    • Decrease or loss of sense of taste or smell

    • Chills

    • Headaches

    • Unexplained fatigue/malaise/muscle aches (myalgias)

    • Nausea/vomiting, diarrhea, or abdominal pain

    • Pink eye (conjunctivitis)

    • Runny nose or nasal congestion without other known cause

  4. If you are 70+ years of age:

    • Delirium

    • Unexplained or increased number of falls

    • Acute functional decline

    • Worsening of chronic conditions
